Mostly because the ignore function exists and everyone's definition of RR material is slightly different.
Do you boot the trolls who are widely ignored or only if the thread gets hijacked... and then do you boot people for simply hijacking a thread because they're an idiot or angry or whatever. And do boot those who don't necessarily start shit, but are constantly continuing it.. and is mocking all of this ok or is that too distracting, too.
It would awesome if there was a magical place where people stated their opinion once and didn't feel the urge keep coming back to say the same thing over and over, and focused more so on what other people thought than megaphoning their opinion.... but, here we are....
