Think about it from this perspective:
Joffrey slowly suffocated to death. There was nothing he could do about it. His last seconds/minute were agonizing, terrible death.
Also, you want torture? Imagine a mother, who loves her darling boy and thinks he can do no wrong, watching the apple of her eye, her first-born and favorite, turn purple and die as she watches, helpless. The boy that she is willing to do anything to protect (including cousin-f***ing, sellsword-f***ing, etc).
