Have you used this combination? Because MLB.TV says you have to authenticate with a cable account. It's not just a location issue, you have to log in with cable account credentials. Does the region unlock not prompt for authentication because it doesn't think the person is in the US? I'd want to know how that works....

Correct. I've used AFT for MLB.TV, NHL GameCenter Live, and NFL Game Pass, and all work great.

I'm not 100% sure of this, but I believe that the need to log in with a TV provider only applies if you're trying to get free content anyway (that you already have access to with your cable subscription). If you're paying for it, you never need to - it's just that stuff shown locally on cable will be blacked out if they think you're in the U.S..

What's some things I should do with it? Install Kodi? Anything else?

installing Kodi is the only thing you need. get it set up properly and you wont need cable, Netflix, hulu, or anything besides an internet connection.
